I believe that this conversation reveals Jezebel's involvement with Jacob is part of his imagined future and that Jacob, when he was alive, knew of Jezebel through work, so she's somewhere within his mind, on a subconscious level. I didn't find the walking up the stairs anticlimatic.

Then there are the two consecutive scenes of Jacob and Jezzie going to the party where imagery of sin lay and the palm reader blatantly tells Jacob he is dead. I think this adds a lot more meaning to Jezebel's character, as she is there to hurt Jacob. In regard to which version of the Jacob’s Ladder story is better, it really depends on personal preference.
I think though the movie is mostly about Jacob coming to terms with his son's death, and achieving that through his own death. As he "lives" just this bit longer the angels in his life protect him from the demons gaining control of him. Jacob's Ladder is a horror classic that inspired numerous modern-day works, but how does the 2019 remake compare to the original story? For instance, after Louie fixes Jacob's back, Jacob has a new hop in his step. As both stories progress, both Jacobs begin to increasingly lose touch with reality as each discover that something more sinister than simple PTSD has happened to them and other veterans coming home from the war. While Jacob’s health initially seems to be fine with the exception of the occasional PTSD-type flashback, the story focuses primarily on his brother Isaac at first. I've always held the belief in the back of my head that the majority of the film depicts Jacob's struggle to reach the afterlife/heaven, and the "demons" he has to conquer in order to do so. Once again the Hollywood remake machine regurgitates another classic for contemporary audiences but botches every attempt to make it unique and invigorating. But they do not really change the fact that you're not 100% sure what is real or imagined and do not give any definitive answers any more than the finished film can.
